Marketing to Hospitals and Nursing Homes

I am putting together a package to send to local hospitals and nursing homes. I would like to include a checklist regarding identification, signature by mark, and determining that the patient/resident is able to understand what they are signing. Does anyone have a checklist or any other ideas?

I don't understand what you are going to include in this "package"
We, notaries, are not attornies and therefore can not offer any legal advice concerning what someone can and can't sign.
Differant states have differant laws as to if or when a witness is needed etc.
