Affidavit of Payment of Taxes

This is to be notarized. At the bottom just above the signatures it states: "This affidavit is given to_______________________for verification of lien related issues.
What goes on that line? TIA

Your only concern is the ack and/or jurat unless the lender instructs you to fill in any blanks. I would contact the agency you are dealing with for this signing.

You cannot, however, notarize it if the document has any blank lines. Therefore, you should contact the lender/title co, etc. to find out what the borrower should write in so you can notarize it.
