I thought I knew which form this was, but I'm drawing a blank as to what it is the signing company want me to send back. Can anyone help?

If you mean FEMA, it could be some type of flood certification

I found what I was looking for. Thanks for your help Bob. It really is a CEMA they were talking about. I guess it's a New York thing. It's a Consolidation Extension and Modification Agreement.
