No idea what this form is as I don't know what state you are in.

BUT is there a notary certificate on it??? If so, then it needs notarizing. Ifthere is not a certificate on it ,are there instructions to notarize it? If so, you need to ask what certificate they want on it, a jurat or an acknowledgment.
