Statement of information....question???

Hello! I just did my first signing! (I know that's a very familiar phrase lately.) But, I would really appreciate it if someone can answer this question that was on the statement information form. Under the part that says "The undersigned declare, under penalty of perjury, that the foregoing is true and correct."

What do you put after "at":
