Just had a few minutes to kill while waiting to get ready for my husband's Christmas Party (it's early this year) and I took a look at your web site. VERY NICE!
I found a form that I've been looking for, the refusal to sign form, and downloaded it. Though I haven't had a refusal in a while, I'm going to add it to the loose forms I carry in my briefcase.

As with any 'good' website, it is constantly under re-construction. In my spare time (when I get any), I try to add things to make it better. I'm glad you stopped by and looked around. The forms are free for the taking.
