New Loan Tax Information Sheet

I know this is for the borrowers to fill out but what is this roughly? Are they to put in information regarding City and State taxes? Most of my borrowers have no idea what information to put on this form. Also, a lot of my signings are done away from the home so they don't carry tax info with them.
