question on loan doc

Has anyone had an assignment of security instrument in their loan package? I have this and am looking it over on how to be filled out. Here is my question, it says Deed of Trust Recorded or Filed on __________________ as instrument/Document No.___________________ in Book_______, Page______________ in the official Records in the county recorder's or clerk' office of blank county.
I leave blank correct? What concerns me, though is on page two there is an acknowledge form to be filled out, but no lines for the borrowers to sign. Explainations please.

This is not for you or the borrower. It is for lender to assign servicing after the DOT is recorded.

Thank you, so on page two also leave the notary acknowlegment, alone since there is noone to notarize.
