Oh that stinks. I'd love to help but that is out of my realm of expertise. Hopefully someone from IN will come on and help.

According the Indiana SOS
You do have a official notary guide. http://www.state.in.us/sos/business/notary/about.html
Follow the link, I am not familiar with Indiana Notorial law, but perhaps this notary guide will give you some guidance as to the useage of this form

I had already gone through that one and it did not provide help, but thank you for taking the time and getting that link for me. I did end up calling NNA and got my answer. I am too fill in INDIVIDUAL in that section. Thank you all that responded.

with your SOS

To assist you in rendering professional service, my office maintains this guide outlining the laws governing the official acts of notaries. Please take a few minutes to read this for your knowledge and protection.
This document, although a good reference, does not provide guidance for every question, problem, or situation you might face as a notary. If at any time you are unsure how to proceed, I recommend you seek legal advice; as a notary you may be liable both criminally and civilly for any negligent or fraudulent acts.

Individual????
What the heck other kind of persons do you have in Indiana. (Yes I understand about the personhood of corporations and all that, but they can't very well "appear before me" now can they)
I'm so confused!

Re: I thought it was Same. -- When is doubt...
Whup it out - attach your own ack for your state.
Janlee, I think "same" or "said" works, but when I run into something peculiar, I just attach a Tx ack and call it a day.

Re: I thought it was Same.
Without knowing how the state requires ack wording, common sense tells me it should be "said".
But, if it was "said" for all....why not just print it in the ack, rather than leaving a blank?
Either that or you could plug in an adjective...... Hairy Person, Rude Person, Nice Person......

Re: Thanks TitleGal, I needed a good laugh this morning...
Every title company I've ever worked for had a "collection" of recorded docs that were just funny, amazing, or of historical note.
A deed from Virgil Earp deeding property in Colton, CA to his wife for the "gentle and loving care for which she rendered after my injuries in Tombstone, AZ..."
A death certificate for a man that died of massive bleeding from injuries to his penis caught in a vise. (yep, it's true)
If someone actually DID write their acknowledgment "..... personally appeared Chuck Blowhard, known to me to be the Annoying Person whose name....." and it got past the recorder?
It would join other recorded docs in the title company Hall of Fame.

I believe the word - "Same" - should be inserted.
