Owners Affidvait As To Mechanics liens, etc..

Can someone assist me with this. I've never seen this wording on an acknowlegement and in VA it is not required.

The foregoing instument was acknowledged before me this 2nd day of February, 2006 by John Doe, the ____________________ of ____________________, as _______________, for and on behalf of the ________________.

I guess I can just use a blank acknowledgement, but if someone knows what goes in those line, I would appreciate your insight.
