Affidavit of lost form???

Do you know where I can get this form?. The car of a friend of mine was stolen and the police want this form from him. Doas he need to go to a lawyer??? Thank you

Probably DMV. Or ask police where to get form.

I think you mean "Affidavit of Loss" which I believe you get from your insurance company.
