How do you notarize this document? Do you attach a separate certificate or write between the lines, the missing info? I was told by many notaries that loose certs sometimes cause rejection if attached to uncle sam's forms!

I don't think a CA notary Public can notarize this form or the DS-11.

It must be done in front of Passport Acceptance Agent at a Passport Acceptance Facility.
