No more is needed. Lots more is often added by lawyers and others who are either trying to justify their fee or their existence.

The Aggie adds the signer's name.

Acknowledged before me on ___ by _________.

Likewise for the jurat.

We don't have a hard and fast rule that our wording must be just so (like California), but to be safe, throw in the basic elements of date and name of signer.
