 Colorado Oath of Office CRS 24-12-101

Colorado revised its oath of office statute, CRS 24-12-101, in 2018, to aggregate and harmonize previous inconsistencies across all statutes involving administering oaths for different public offices. The oaths of office must now be in writing and signed, with specific wording.

The SOS has posted a Constitutional Oath of Office Form with a notarial certificate on their website at https://www.sos.state.co.us/pubs/business/PDFFillable/OATH.pdf

CRS 24-12-102. Whenever any person is required to take an oath or affirmation, other than an oath for public office or position in accordance with CRS 24-12-101, the person shall take or subscribe the oath or affirmation in the manner specified in the particular law that imposes the requirement.

So, the notary can check to see if there is a form required by law or available from the relevant agency.

If not, I have a simple form for Oath or Affirmation for Witness Testimony with a normal jurat notarial certificate. It is just the verbal oath of the witness to tell the truth, in printed form.
