This kind of thing is one reason why I get a bit anal about cross-referencing loose certificates (or any that don't include signers' signature lines, doc title, page number, bar code, etc.), making sure pronouns are indicated and no space is left to add any names in the certificate.
I do think it's very rare for people to intentionally set out to commit fraud. But "getting creative" to adjust for mistakes probably happens much more often than we'd like to think. |
