NO signatures lines for the borrowers

The refi package for my signing today did not have signatures lines for the borrowers on either the Mortgage or the PUD Rider!

It was a last minute signing. I received the call just after 10:30 AM for a noon appointment time but did not receive the documents until almost 11:30. I confirmed with the borrowers for 12:30, printed, and made the 30 minute drive to the borrowers' home. As we were making our way through the pile of documents and reached the Mortgage, I noticed that the only signature lines were for witnesses...at least all the other docs appeared to be formatted correctly.

Has this ever happened to any of you?
