although your post says TN I believe you're in AL??

Here in FL I could complete my cert with "Bernice A. Former a/k/a Bernice Later" with ID only for Bernice Later and her representation to me that she was formerly known as Bernice A. Former. Check to see if you can acknowledge a/k/a's in your state based on their representation to you.

Then, you address this: "Please correct and resend that page.*"

No can do - return the entire document to me and I'll have to revisit her to re-acknowledge - I cannot provide a loose replacement cert. Can you do that in your state?
