You haven't provided enough information to say if the government office was right or not. Was it an acknowledgement (if so, they're wrong). Was it an oath or affirmation (if so, they're right).
Was it one of the forms available at https://www.cdph.ca.gov/Programs/CHSI/Pages/Vital-Records-Obtaining-Certified-Copies-of-Birth--Death-Records.aspx ?
Those forms say a sworn statement is being made, but use language in the signer's section appropriate for an affirmation. Then there is a notarial certificate for a California acknowledgement, so we can safely conclude the form was drafted by an idiot.
Options:
1. Resubmit the form, with all dates the same.
2. Find out which elected official is responsible for the California Department of Health, and picket the person's home until they get it fixed.
3. Spend $50,000 to sue the California government to compel them to accept the original form. |
