 Chase Notarial Evidence form

I just read trhu a bunch of forum comments on this, and I may be blind, but I did not see if anyone had used it , and just listed the Notarized Chase docs, and not LSI's Notarized docs, and found that to be OK. I have my first job with the new form tomorrow.
