 Did my very first acknowledge?

I hope I did it right. I didn't have my workbook with me so I played it by hear. Should I sign the document as well as stamp it with me notary stamp? I only stamped it and followed the instruction in my journal. Feeling a little worried I did something wrong. Please someone comfirm with me did I mess up.
