 Wording help

I need some help with wording. I have a client who wants me to Notarize a POA. Other than me signing and stamping what word do I write on the POA? Something like: Befor me _____________ a Notary for the state of VA and the Couty of Chesapeake attest that __________________ appeared before me with suitable identification.

I have done hundreds of loans but everything was alreday typed in and I am having a senior moment DAH! Any help would be appreciated.

Thanks, Violet /VA

Gee, I wonder how many ugly remarks I will get for asking this!!!!!!!!!!!!!!!!!!!!!!!!!
