The AKA form has nothing to do with you properly identifying a signer with your notary hat on. That must be done in compliance with your state's notary law, separately from anything on the AKA form, which is strictly for the lender. [And you haven't let us know what state you're in - unless you ARE in Alaska...? Wink]
