So what you're saying is (correct me if I read this wrong):
1) You are "notarizing" and unsigned document? First, documents are not notarized, signatures are. So, you cannot perform the notarial act until the document is signed by the person or persons in front of you. And how do you handle the jurat? They must swear to an oath or give an affirmation. Then you complete the jurat.
2) Do you simply say "sign here"? You don't tell them what the document is for?
