I've seen them without dates lots of times. But like you said, the form says they aren't supposed to be that way. This varies from one lender to the next. It might even vary from one borrower to the next, depending on their qualifying circumstances, but that's just a guess.

Since we don't do anything with this form other than present it for signature (and make sure the box gets checked), I'd stay as neutral and possible. Having the borrower call the lender is the right thing to do, but I wouldn't offer an opinion either way, even if asked. I get asked about lots of different stuff all the time that I don't think is appropriate to comment on and that's pretty much what I tell them - or say that it varies, etc.
