| Thank you guys for responding so quick. I should have mentioned that I had previously called NNA who said in PA, I need to line out the AKA info or attach a loose cert. I also looked at PA regs and didn't find specifics so I called the State. They were a little vague on their answer and said they won't give advise but as a PA notary, we are required to make sure "the individual appearing and making the acknowledgment has been properly identified".
