Not just lenders or title companies. The form from the California Department of Health for a certified birth record has a heading over the area where the applicant signs that says "SWORN STATEMENT" and requires the applicant to "declare under penalty of perjury under the the laws of the State of California". But the preprinted notarial certificate is a standard California all-purpose acknowledgement certificate.

This could apply to me because in my volunteer work I assist people who's homes have been destroyed by fire. I may be called upon to assist them in replacing identification materials, including birth certificates. I've encountered fire survivors who were born in CA, although they didn't need their birth certificates replaced.
