I think I understand your question.. The jurat that is preprinted on the form does not have the exact wording required by your state, correct? Since I am not in your state, I cannot give advice. I did see this subject discussed not too long ago. Have you tried to search on Jurat.
In my state, if the verbiage is incorrect, I will attach a loose jurat or acknowledgement. |
