From my experience, no they wouldn't be listed on the 1003 if they are a housewife or self employed and they didn't want to use that persons income to get the loan.
The loan application is all the information the lender used to put the loan package together.
Key items I look for: If one spouse is not on the 1003 then are they listed on the note or the mortgage. Now I strongly URGE YOU to keep in mind I am speaking from experience and my state guidelines. Are you in what is called a spousal state?! THIS IS VERY IMPORTANT!
