A loose acknowledgment should ALWAYS contain information identifying the document it is prepared for; otherwise, it's literally a "loose acknowledgment" that can be attached to ANYTHING. I always carried a supply of loose acknowledgments on both letter and legal paper so that they would match the paper size of the document, and would use those if needed even if the docs came with vendor-supplied loose acknowledgment.
The bottom section contained spaces for information about the document itself - what kind of document it was, any identifying dates or serial numbers, etc. The goal is to totally tie the acknowledgment to the document, even if the staples were removed.
NNA sells pads of these in letter size, but why pay for something that will take you 5 minutes to create with a word processor? I printed both letter and legal versions, because some recorders get bent out of shape if the page sizes don't match... |
