You could try a bricks & mortar store such as Staples. You might not be thrilled with the version you can find, but it will get you through. Failing that, you could get any bound blank book with pre-numbered pages, and use an old journal to remind you of what to enter for each appointment. You might find something in the section with blank books for accounting.
