It's pretty much Notary 101 anywhere that the elements of any notarization include the signer being either acknowledged or sworn in to the content and/or truthfulness of the document, as well as date and venue. Otherwise, what are you notarizing?

If no wording appears, you have to ask the signer which wording they wish to use, ack or jurat - you can't decide for them.
