We have something like that here on our criminal records check, drives me nuts, since it is a state doc.
They even ask for a seal.
Called the SOS on it and they said to just attach something.
We can now witness a signature and that is what we are doing so I attach that form.
To me it is a waste of time and money for the signer if they ( the state police ) insist on a notary signature and stamp and do not even provide the proper wording. |
