Not going to get into the UPL question.

The POA should already have the wording for a jurat. (If not, it must be a home made POA and a real dangerous document.)

You just notarize the jurat section, if witness are required, have them sign.

Nothing real special - just another notarization.

Try search for "legal forms" with the search button. Probably can find website mentioned in postings where you or your client can see the wording for a POA.
