Has anyone gotten a receipt from them for payment?

However, I am wondering what other companies you all have heard from asking us to be gouged by you know who, in order to work for them.
Previously, I was wondering why I was not hearing from LSI as much lately, and now I know. My fees are too high, but lowering to $50.00? WTF? |

I have called, and left voicemails, as everyone I speak to has no idea how I will get paid. I would like to just fill out the statement and Fax it, but it does not have a Fax # on it.
Any help would be appreciated.
