Who's Signs this Doc?

I am new at signing and have an "Endorsement Allonge to Note" in the doc package, but am confused on if the borrower signs this or if I do or if a settlement agent does? I am confused mainly because below the signing line it asks for the person's title, which leads me to question who should be signing this doc. Any info would be appreciated. Thank you.
