invoice/cover page

Do I need to have my own cover page or my own invoice page, (whichever is relevent) or does the company provide that?

Each company have their own requirement. Some do not require invoicing, some only want one invoice made up per month, etc. You need to read the contracts of each company.

I make up an invoice for every assignment, for my record. I do not submit it to anyone if it is not required by the company or if only monthly statements are required. Keeping a pile of invoices in my receivables file helps me keep track of whats open.
