How should this be notarized (question of capacity?)

On _________, 20 _____ before me, ___________ personally appeared JOHN BORROWER and JANE BORROWER, John Borrower and Jane Borrower, husband and wife as joint tenants, yada yada yada.

Should I :
cross out: husband and wife as joint tenants,
leave it as is,
cross out all of the verbiage and attach an ACK
or ....

Also on the bottom half of this DOT Certificate of acknowledgment is request for full reconveyance.

What is this and does this need to be filled out?

This is a problem with software on digital doc programs I think. They plug in how they are taking title and it carries over into everything in the docs, including the acknowledgement.

When I'm signing I cross out "HWJT" and initial it. When I'm recording docs provided to me in my day job, ack'd by another notary, I leave it alone. To date, the "HWJT" in the ack has never caused a problem at the recorders office (in my county in CA). So...my conclusion is it doesn't matter.
