At the bottom of an I.D. Affidavit where the notarial wording reads:
Attorney_______________
Sworn to b4 me this ____ day of _____
Notary public for ____________________
My Comm. exps. _____________
Do I cross out the attorney and print Signing Agent?
What goes after the FOR?

I would attach a loose Jurat. I am in Califonia the wording is all wrong for here. What state are you in?

You need to let us know what state you are in.
In Florida you would put
Example: Notary public for the state of Florida county of Duval
I would cross out Attorney and put Notary not Signing Agent but this is what I would do, I am not an Attorney and this is not legal advise.

In the line where they say attorney, I would put the name of person being notarized. as it is followed by swore and subscribed before me. Correct?

Under the attorney line I would put my name and behine it write notaty. and fir the state of ... where ever you are
