Unusual Venue

In Vermont a person may deposit a will with the county courthouse for safekeeping. To get it back, one must submit a notarized request, and I just did a jurat for one of these requests. The pre-printed venue on the jurat was rather unusual:
: State of Vermont ss.
: District of Washington
:
: At ____________________ before [Notary's name] personally appeared [Affiant's name] . . .

The court staff said to leave the preprinted venue alone, since the courthouse is in Washington County, and put the county where the oath was administered after the word "at", which is what I did.

Has anyone seen anything like this before?

(for those wondering what I charged, I didn't. I don't charge while I am at my teaching job.)
