acknowlegdegment question

hello, I have an acknowledgement that is worded in a way I've never seen before and want to get your help in filling in accurately. thanks and here it is.

STATE OF_____________
COUNTY OF ___________

This instrument was acknowledged before me on _____,______2005, by _________. (in the rare case of a Corporation of Trustee it will contimue "in my capacity of ______ for____.")

The bottom part is what I've never seen before, and don't know how to proceed with

In the rare case that you are notarizing for a Corporation or (yeah, that 'of' should be 'or') Trustee.... worry about it. If it's just individuals...don't worry about it.
