I was ask to notarize a I-134, Affidavit of Support

I'm going to attach a loose jurat, but my client was telling me that he need fax the document.
What I need to do?

If the language you posted is the jurat, you do need to attach the correct language with a loose certificate, as you said.

Are you asking how to fax it when it's attached? Just take the staple out first. Is that your question?
