Having brain cramp...please help.

On (date), before me, the undersigned, a Notary Public in and for said County and State, personally appeared (John Doe and Jane Doe) proved to me on the basis of satisfactory evidence to be the person(s) whose name is/are subscribed to this instrument, and acknowledged that _____________________ excuted the same. What do I need to put in that blank?
