Help! Corporation Assignment of Mortgage

Does anyone (in Indiana, if it matters) know how this form is to be notarized? I ran across one a long time ago, and it had a name other than the borrower, and it had been notarized already.

The loan officer/title co. instructed me to notarize it with the borrowers' names in it.

Any guidance would be greatly appreciated.

That's usually used for sale of the mortgage on the secondary market & it does not get notarized by us. If the borrower's names were inserted, who would they be selling their mortgage to???

Thanks! I told the LO that it did not pertain to me, but he insisted I notarize it anyway. I wasn't going to, so thanks for the second opinion!
