Need Help filling out a form

Hi All,

Can someone help me fill out this acknowledgement certificate? It states:

On this ___ day of ____, 20___, before me, the undersigned notary public, personally appeared _________ (name of document signer), proved to me through satisfactory evidence of identification, which were ______, to be the person whose name is signed on the preceding or attached document and acknowledged to me that (he) (she) signed it voluntarily for its stated purpose.

(as partner for________________________, a corporation)
(as__________________for______________, a corporation)
(as attorney in fact for _____________________, the principal)
(as___________for___________, (a)(the)__________)


I get filling in the date portions but the rest is a bit confusing. I haven't had to fill this form out before. Any help is greatly appreciated.

Okay - is this your normal MD certificate?

If not, you can use that, b/c you are familiar with that. Print one up off your computer and you'll know exactly what to do.

state's forms on page 15 / 16 / 17 of your manual. Just type one up on your computer, print it out and keep a stack of those handy.

BTW - What kind of document is this one you have attached to???

Thanks! Actually it needs to be attached to the deed of trust. This is a form that I have my self. Here is what it looks like: http://www.notarysuperstore.com/level3.cfm/Search/CatSearch/LevelOne/Notaries/LevelTwo/Forms%20&%20Certificates/LevelThree/Acknowledgement.

IMHO the correct acknowledgement will be in your notary handbook. I cannot tell you what is correct or not. I always make my acks specifically from the notary handbook of Texas.

Am I missing something here? MD requires a TPL...

and this is a pretty basic ack. that I would think someone holding a TPL would not have a problem completing.
