Credible Witness Acknowledgment

can anyone from California suggest proper wording for a Credible Witness Acknowledgment?

Do you solemnly swear that ( Miss Piggy) is
1. The person named in the document?
2. That( Miss Piggy) is personally known to you?
3.That it is your reasonable belief that the circumstances of ( Miss Piggy) are such that it would be very difficult or impossible for her to obtain another form of identification?
4. That ( Miss Piggy ) does not possess any of the identification documents?
5. That you , the credible witness have no financial interest nor are you named in the document being acknowledged.
UNDER PENALTIES OF PERJURY? OR SO HELP YOU GOD?

You do not do a credible witness acknowledgement. Enter the witness information into your journal, get all the information you can on them, have them sign,fingerprint and give them the oath that they know the borrower and this sworn statement acknowledges their being.

You do not need to get a fingerprint of the credible witness. Check the IDand enter it in your journal, administer the oath, and have them sign your journal in the ID section. The ack is the same as any other ack.
