Nursing Homes

Does anyone have a list of questions they ask when visiting patients in a nursing home setting to determine their awareness?

Here's a list that an attorney I know asks. However, I just engage the person in conversation about the weather and the room and people in the news and casually ask some questions here and there. It's a friendlier way, and I get less hostile responses and a good read on competence.

Attorney's List:
State your full name and date of birth.
What is your current age?
State your current complete address.
What is your marital status?
State the number of children you have and their complete names.
What is the current dat and date?
Who is the President of the USA?
What country are US soldiers currently fighting in, Iran or Iraq?

If son or daughter in room ask them what is thier brithday / who is Pres.
what day of week / The upmost do they know what they are signing .
I had one the children was just trying to get his money . Be careful
