Name affidavit
This may be a stupid question, but I have a signing to do this afternoon. Just received docs. Ther's one doc that is listed as Name Affidavit reads as follows: This is to certify that (borrowers name), is one and the same person as( ) who executed a mortgage/deed of trust to (bank name)

what goes in the 1st blank parenthis section, and am I to sign it just as a witness?
Any help would be greatly appreciated. I'm fairly new to the business, and this is the first one of these docs I've seen so far

Ah hah!! did one yesterday like that were the borrowers names are on all loan documents as "Jon Doe" but also known as "Jon A Doe" all at the top and then it goes on to state that I (agent) certify that this person is also known by that name???? Well, here is what I did. Because of needing the photocopy of ID and on their ID it did give their middle names I could verify that they were their "initial' by looking at the ID. Since you are going to be sitting there with them on that you need to affirm with them that this is correct and then I just input my venue, and the wording...Sworn to (or affirmed) and subscribed before me this ___ day of _____, 2004 by "Jon Doe", and producing evidence of DL#....... issued on ___/_/___with expiration of ____ . and then I was able to affix my seal as needed.
Sometimes that wording will throw you off especially if they don't have it there and in paranthesis ( attach jurat or proper notarial documentation here) and I do not and will not attach a loose leaf notarial certificate if I can find another way of doing so.

that document had me confused too since i knew that it seemed to be pertaining to me and I cannot notarize my own statement!
There is the other document in there for the Patriot Act where you go and look at their original IDs and filled that paper in and then at the bottom of it it states for notary to certify and sign and affix seal.. well with that one, I just let them know that I could not legally notarize my own statement and could not affix my seal to it! On one of the cover sheets with the packet it stated to signed and affix seal to that document but I could find no justification in doing so without breaking the law and leaving myself wide open for problems.. so noted on that same sheet my reason for not doing so and also informed through my status report to signing service of my reasons for not doing so.

Good Luck!! It seems that so many of the documents that you have looked at and went through before seem to change right before you eyes! When you think that you have seen all versions and wordings within them they throw a new twist in them!!! Geesh... no wonder my toes are sore.. from staying on them!!! LOL...
When in question about anything that I have and don't have time.. I call the experts!!!
