Authorization to close account

this has the address and etc., and the account no.# and company to which the account is to be closed. What do I do here. The couple is refi to pay off a huge debt. Many account no.#'s involved. Have couple to sign and leave blank?

Some companies just put one in the package for the borrowers to sign, then make copies, fill in the account numbers & amounts to close out.
