Preliminary Change of ownership report

Is anyone familiar with this California document and do we have the buyer fill this out?
There was one in a package today but nothing in the instructions or doc list about it.

Any help will be greatly appreciated.

This document is something that the title co. fills out for the borrower. When I have asked about this document they always tell me to just have them sign it and they'll fill out the rest , if the borrower is unsure on how to fill it out.

It is a document that tells the recorder why the property is having a change in vesting. You will usually only see it when there is a Grant Deed, Quitclaim, or the like. If it is not recorded with the document, the recorder will charge an additional fee to cover the cost of them sending it out to the borrower for completion. The title company will usually fill it out for the borrowers and I just have the borrowers sign.

This document has created 2 quality hits from FASS. I request the borrower to complete to the best of their ability and note that in my wrap up to the SS and still I get a hit on it.
