HOW TO: NON-COLLUSION AFFIDAVIT in CA?

Hello. I am a new notary public in California. Can someone please provide a guide in how to complete a NON-COLLUSION AFFIDAVIT. Thank you.

An affidavit is an affidavit is an affidavit. In CA if it does not comply with CA law (in other words if the wording is different than CA wording) then attach a CA all purpose form. JMHO. Anyone else?

You don't "complete" any document. The document is to be completed by the document preparer and/or signer. You complete the notarial section, as any other document you'd notarize.
